This form is available for reporting discriminatory or harassing incidents or behaviors that may be potential violations of the Chi Phi Human Dignity Policy and/or Policy on Transgender/Non-binary Inclusion. Reports can also be filed directly to the International Headquarters of Chi Phi 404.231.1824 or chiphi@chiphi.org
